Coccydynia is pain arising from the coccyx, or tailbone, at the base of the spine. It can follow a fall, childbirth, prolonged sitting, direct trauma, pelvic floor dysfunction, joint stiffness, coccygeal instability, muscle spasm, ligament strain or nerve sensitivity. NHS guidance notes that physiotherapy may involve manipulation of the muscles around the coccyx to help ease pain.
Tailbone pain can be sharp, aching, bruised, burning or pressure-like. It may be worse when sitting, rising from sitting, leaning back, cycling, driving, bowel movements or after long periods of pressure.

The first appointment includes full medical history, examination, diagnosis, digital X-ray if needed and fully consented, first treatment and advice. Treatment may include external coccyx and pelvic work, soft-tissue therapy, pelvic floor-related treatment, sacroiliac and lumbar care, massage, posture and sitting advice.
Where clinically appropriate, internal coccyx manipulation may be discussed. This is only performed with full explanation, explicit consent and strict professional care. The aim is to assess and treat coccygeal mobility, pelvic floor tension, soft-tissue restriction and mechanical irritation contributing to pain.
